Interlude at Abbott Northwestern – WestHealth in Plymouth

Administrator Kayla Wagner, LNHA, holds a B.S. degree in Health Services Administration with an emphasis in long term care from Minnesota State University Moorhead and is a licensed nursing home administrator. She has five years of health care experience in metro skilled nursing facilities and post-acute centers.


Clinical Administrator Sharon Kern, RN, has over 30 years of nursing experience. She has worked in all facets; from acute, long term and home health care, giving her a broad knowledge of policy and regulation management experience. She has spent the last 20 years working in long term care where she was the ADON for a 350 bed facility. She is a graduate of  Gustavus Adolphus College.

Rehabilitation Program Manager Joely Wertz, MS CCC/SLP, has utilized her background in communications to work effectively with patients in a variety of settings, from outpatient to long-term care. She received her Masters of Speech-Language-Hearing Science from LaSalle University in Philadelphia, Pennsylvania and obtained her undergraduate degree in Communications, with a concentration in Health Care from Marquette University.

Chief Medical Officer Dr. John W. Mielke brings a significant and unique blend of experience in health care quality, physician/patient and family relationships, geriatric health management, innovation and patient safety. He received his medical degree from the University of Minnesota Medical School and completed his residency at Abbott Northwestern Hospital.

Interlude at Mercy Hospital - Unity Campus in Fridley 

Administrator Nicole Donahue, LNHA, holds a B.B.A degree in health care administration from the University of South Dakota and is a licensed nursing home administrator.  She has over eight years of health care management experience operating rural and metro skilled nursing facilities and post-acute centers throughout Minnesota. 


Clinical Administrator​ Nate Dahle graduated from University of Mary-Bismarck with both a Masters in Nursing Administration and MBA in Healthcare. He has been a nurse in long term care for almost 10 years with 8 of those years serving as a director of nursing. In addition, Nate spent a year and a half in the hospital as a floor nurse and infection control/performance improvement coordinator and was a Presbyterian Homes employee for two and a half years at Presbyterian Homes of Bloomington.  

Rehabilitation Program Manager Deanna Hughes received her BS in Occupational Therapy from the University of Wisconsin Milwaukee, graduating Magna Cum Laude. She has been with Presbyterian Homes & Services for 8 years and in leadership for 7 of those years. Prior to transitioning to Interlude, Deanna worked at Johanna Shores as a Rehab Supervisor and transitioned to Langton Place in 2013 as Rehab Program Manager.


Medical Director Dr. Neal Buddensiek has been a diplomat of the American Board of Internal Medicine since 2006. He obtained his doctorate of medicine from Creighton University.  His experience includes hospitalist at Abbott Northwestern Hospital as well as both TCU physician and TCU medical director at Allina SeniorCare Transitions. Dr. Buddensiek also currenly serves as cheif medical officer for the Benedictine Health System.
